When something urgent happens, the last thing anyone wants is to hunt for a resident’s key information. A resident face sheet — and a ready emergency packet — puts the essentials in one place.

What a face sheet commonly captures
- Resident identity and key demographics
- Diagnoses, allergies, and code status
- Representatives, guardians, and financial contacts
- Primary provider, preferred hospital, and pharmacy
- Insurance details
Keep an emergency packet ready
Many homes keep a printed face sheet and emergency contact list where they can be grabbed quickly — for example, if a resident is transported. Keeping these current means the information that leaves with the resident is accurate.
Protect sensitive details
Face sheets contain sensitive information, so store them securely and share only what’s needed. Some operators deliberately avoid recording full Social Security numbers on documents that travel.
