1. Overview & Agreement
These Terms of Service ("Terms") govern your access to and purchase of digital document templates from CareForm Pro. CareForm Pro is a brand of Merchant Supply Group, LLC, a Washington limited liability company ("we," "us," or "our"). By accessing our website, purchasing products, or downloading content, you agree to be bound by these Terms.
2. Digital Products & Single Facility License
CareForm Pro sells downloadable Microsoft Word (.docx) and PDF document templates designed for Adult Family Home (AFH) operators. Upon completed purchase, you are granted a non-exclusive, non-transferable, single-facility license to use and edit the templates for one Adult Family Home location that you own or operate. Multi-home operators must purchase additional licenses or contact us for enterprise licensing.
You are strictly prohibited from reselling, redistributing, publishing online, or sharing these document files with third parties or competing template distributors.
3. Orders, Pricing & Checkout
All transactions are processed securely through Lemon Squeezy Merchant of Record. Prices are listed in US Dollars. We reserve the right to modify prices or discontinue products at any time without prior notice.
4. Intellectual Property
All site design, text, graphical layouts, paper-stack card visuals, and document template structures remain the intellectual property of Merchant Supply Group, LLC. Your purchase grants operational use rights but does not transfer copyright ownership.
5. Limitation of Liability
CareForm Pro, Merchant Supply Group, LLC, and its representatives shall not be liable for any regulatory citations, fines, state audit findings, or operational disputes arising from the use or misuse of our templates. You are responsible for ensuring all customized forms comply with local Washington State regulations.
