However, as an AFH provider, you must insert your home's specific legal entity name, operating policies, and state-specific license details. We recommend having your legal counsel review customized contracts prior to execution.

Every template includes both fully editable Microsoft Word (.docx) documents and printable PDF reference files. You can open and edit the .docx files in Microsoft Word, Google Docs, Apple Pages, or LibreOffice.
