No. CareForm Pro is an independent document template provider. Our templates are designed to help Adult Family Home operators maintain organized recordkeeping, but they are not official government forms, nor are they pre-approved or endorsed by DSHS or any state regulatory body.

"Inspection-ready formatting" means the document is professionally organized and easy to review. It does not mean the template has been approved by a licensing agency or guarantees compliance.

Every template includes both fully editable Microsoft Word (.docx) documents and printable PDF reference files. You can open and edit the .docx files in Microsoft Word, Google Docs, Apple Pages, or LibreOffice.

Immediately after completing checkout, you will see a download button on screen. Additionally, an automated receipt email containing your download links will be sent to your email address by Lemon Squeezy.

Our templates are reviewed periodically for formatting, structure, and operational clarity. If you purchased a template and an updated revision is released within 12 months, you can request the latest file version at no additional charge by emailing hello@careformpro.com.

Each purchase grants a single home license for unlimited internal use within your facility. If you operate multiple Adult Family Homes, please contact us for multi-home bundle pricing. Re-selling, redistributing, or sharing files with non-purchasing providers is strictly prohibited.

Microsoft Word gives you the best editing experience, but you don't strictly need it. The .docx files also open in Google Docs, Apple Pages, and the free LibreOffice — and every template includes a print-ready PDF version as well.
